About the Link
The Allegheny Link is a program in Allegheny County funded by the Pennsylvania State Office of Long Term Living. The mission of the Allegheny Link is to simplify and streamline access to long-term living services and supports and provide assistance to consumers who are seeking services and making long-term living decisions.
This is a collaborative effort among local agencies, organizations and individual consumers.
The Allegheny Link provides a wide array of services to people of any age with a disability, anyone over the age of 60 with or without a disability, their advocates and professionals in Allegheny County.
These services include:
Information and Referral
Eligibility Screening - assisting consumers determine what programs and services for which they may qualify
Options Counseling - assisting consumers decide what programs and services would best fulfill their needs
Application Assistance - helping consumers in the completion of any paperwork necessary to apply for programs and services
Case Management - available to consumers with disabilities ages 18 – 59
Follow Up - contacting consumers after a predetermined time interval to ensure that they are receiving or in the process of receiving programs and services for which they have applied
