About the Link
The Allegheny Link is one of two pilot programs
in Pennsylvania funded by the Governor's Office of Health Care Reform
in collaboration with the Pennsylvania Department of Aging and the
Pennsylvania Department of Public Welfare with a grant from the
Centers for Medicare and Medicaid Services and the Administration
on Aging.
The mission of the Allegheny Link is to simplify
and streamline access to long-term living services and supports;
provide assistance to consumers who are seeking services and making
long-term living decisions; and to increase awareness and provide
reliable information.
This is a collaborative effort among local agencies,
organizations and individual consumers.
The Allegheny Link provides a wide array of
services to accomplish our mission. These services include:
Information and Referral
Eligibility Screening - assisting consumers
determine what programs and services they would qualify
Options Counseling - assisting consumers
decide what programs and services would best fulfill their needs
Application Assistance - helping consumers
in the completion of any paperwork necessary to apply for programs
and services
Follow Up - contacting consumers after a
predetermined time interval to ensure that they are receiving or
in the process of receiving programs and services for which they
have applied
