Michael Dallas Waiver

The Michael Dallas Waiver provides home and community based services to eligible persons of any age who are technology-dependent allowing them to live in the community and prevent institutionalization.  Technology dependence is defined as requiring technology to sustain life or replace vital bodily function and avert immediate threat to life.

Target Population

Individuals who are technology dependent (i.e. requiring technology to sustain life or replace a vital body function and avert immediate threat to life.)

Eligibility Criteria

  • Individuals of any age who are technology dependent
  • Income limit must be equal or less than 300% of the Federal Benefit Rate
  • Individuals must meet the level of care for Special Rehabilitation Facility
  • Waiver staff make a minimum of one visit to the home of the consumer to complete the evaluation for waiver services
  • Total countable resources for persons over the age of 21 must be $8,000 ($2,000 Medicaid limit that includes a $6,000 resource disregard) or less.  Resources include, but are not limited to, cash on hand, bank accounts, stocks, bonds, mutual funds, life insurance and vehicles.

Cost Sharing

There is no cost to the consumer for waiver services.
