• Claims Form (No.93) - To file a claim against an estate.
• Guardian's Inventory and Statement Form – To file an inventory for the estate of an incapacitated person.
• Inheritance Tax Forms are available at the Department of Court Records, Inheritance Tax Department located in room 216 City County Building, or you can download them from the PA Department of Revenue.
• Civil Commitment Records Request Form - Obtain personal civil commitment records from the Orphans' Court.
