Report A Concern
To report about the safety or well-being of someone or have a concern about a Department of Human Services (DHS) service or provider, there are several ways to report it.
Concerns about DHS or DHS Providers
Questions, complaints, or concerns about any service provided by DHS or a DHS-contracted provider can be issued through the Director’s Action Line (DAL) at 1-800-862-6783 (Monday to Friday, 8:00 am to 5:00 pm). Concerns about suspected Abuse or Neglect
Reporting Child Abuse or Neglect (For anyone under 18 years old)
To report a concern about the safety or well-being of a child, please call 412-473-2000 or 1-800-932-0313. These numbers are available 24 hours a day, 7 days a week.
When calling, reporters may be asked for:
- The child’s name, age, and home address
- The name of the child’s primary caregiver
- The concerns that prompted your call
Reporters may remain anonymous and do not have to give a name or contact information.
Mandated reporters (such as teachers, doctors, and social workers) should use the PA Child Welfare Portal to submit reports online.
Reporting Abuse or Neglect of Adults with Disabilities (Ages 18-59)
Concerns about the safety of an adult with a disability should be reported to 1-800-490-8505. This hotline is available 24 hours a day, 7 days a week.
Report any suspicion of:
- Abuse
- Neglect
- Exploitation
- Abandonment
Reporters may be asked to provide:
- Your name and contact information (optional)
- The individual’s name, age, and address
- A description of the concern or incident
- The name of the alleged perpetrator (if known)
- Any safety or health concerns
Reporters may remain anonymous. Reporters are protected from retaliation, discrimination, or prosecution.
Reporting Abuse or Neglect of Older Adults (Age 60 or older)
Concerns about the well-being of an older adult, please call 412-350-6905 or 1-800-490-8505. These phone lines are available 24 hours a day, 7 days a week.
Abuse reports can be made by or on behalf of an older adult who:
- Lives in a private residence
- Spends part-time in adult day care
- Lives in a care facility (nursing home, personal care home, hospital, etc.)
- Is experiencing homelessness
A Protective Services caseworker from the Area Agency on Aging will investigate reports of:
- Abuse
- Neglect (including self-neglect)
- Abandonment
- Financial exploitation
Reporters of abuse or neglect of an older adult:
- May remain anonymous
- Are legally protected from retaliation, discrimination, or prosecution
Reporters may be asked to provide:
- Your name, address, and phone number (if not anonymous)
- The older adult’s name, address, and phone number (if not self-reporting)
- A description of the concern or situation
- The name of the alleged perpetrator (if known)
- Details about the individual’s physical or emotional health and any safety concerns
