Meeting Special Needs

Professional social work staff assist residents and families throughout their stay. Trained in the most up-to-date elder care skills, our staff eases concerns about insurance coverage, discharge arrangements and networking with community services.

Our social services staff members work with residents to:

  • Identify their psycho-social needs
  • Provide, develop, and/or aid in the access to services to meet those needs
  • Educate resident and family relative to illness, social and emotional needs
  • Advocating with and for our residents to ensure greater choice, quality of life and quality of care. This may include consultation with Ombudsman.
  • Coordinating and leading quarterly care plan conferences
  • Assist our residents, families and friends with grievances.
  • Direction of discharge planning and follow-up with our residents, families, interdisciplinary team and community service providers.
  • Participating with end of life planning, including legal and health matters.

These staff members are part of the multi-disciplinary team which fosters a climate , and develops policies and routines to enable our residents to maximize our residents to maximize their individuality, independence and dignity. This positive climate provides residents with the highest practical level of physical, mental and psycho-social well being and quality of life.
