There are four ways to get started with WIC:
- Visit the PA WIC website and complete an online pre-application
- Print out a paper application (see "WIC Applications" below) and take it to your health care provider
- Call 412-350-5801 to request an application through the mail
- Stop by a WIC clinics to pick up a paper application
Currently, WIC serves over 11,000 participants in Allegheny County alone, with available funding to support many more applicants. Anyone who is potentially eligible is encouraged to enroll in WIC. Don't delay, apply today!

Qualification Guidelines
All WIC applicants must meet income guidelines and nutritional/medical risk criteria. These standards are set by the US Department of Agriculture.
The following are the current income guidelines. They are revised annually.
Unborn babies count towards family size. Foster children, LIHEAP, TANF, and food stamp clients are income eligible for WIC.

For each additional member add $10,175.
Note: If someone is on LIHEAP, SNAP (Food Stamps), or TANF, that person automatically meets the WIC income guidelines. Your unborn child counts as a family member. A custodial father, foster parent or legal guardian of a child up to age 5 may receive WIC benefits for the child.
Nutritional/Medical Risk
Medical/nutritional eligibility is determined by a WIC nutritionist, based on the information your health care provider gives us as part of the application.
Nutritional risk is any health condition that may be related to improper diet habits, poor growth, or medical conditions that are influenced by your diet. Some examples of medical conditions related to what you eat include:
- Low iron levels in the blood (anemia)
- Being underweight
- Being overweight
- A history of substance abuse
- Recent major surgery
- A history of pregnancy complications
- Chronic disease, such as diabetes
Applications
If you'd like to apply for WIC benefits, the first step is filling out an application. Both you and your health care provider must complete the form.

What Should I Bring to My Enrollment Appointment?
Please bring the following to your first appointment:
- Proof of identity, for example:
- For a woman – driver’s license, photo ID card, passport or student ID
- For an infant – birth certificate, crib card or discharge paperwork
- For a child – birth certificate, crib card or passport
- Proof of all sources of income for everyone living in the household (ex. pay stubs for the last 30 days, unemployment letter, child support, income tax return for self-employed persons only)
- ACCESS card (if applicable)
- Proof of Pennsylvania residency (Any document with name and current address such as driver’s license, utility bill, or recent mail)
Immunization record for all children under 2 years old (ex. baby book, health passport), if available
- Your child(ren) under age 5
- Proof of pregnancy stating due date (if applicable)
- If possible, complete a dietary questionnaire for each applicant:
If you are currently on WIC or are acting as a proxy for a WIC participant, always bring your WIC packet and WIC ID card to every appointment.
For re-certifications, please bring:
