File a complaint or ask a general question on the Allegheny County Self-Service Portal. Select Request Service/File Complaint, then select Health Department, and then select the appropriate program.

Speaker Request

If you are an organization, school, non-profit, etc., and you would like to have a representative from the Health Department speak or provide a lecture at your event, please complete this ACHD Lecture and Speaking Engagement Form and someone from the Health Department will contact you.

Event Attendance Request

If you are simply interested in having a Health Department representative attend your event, please fill out this ACHD Event Form and someone from the Health Department will contact you.

Hosting a Vaccination Event

If you are interested in hosting a vaccination event in coordination with the Allegheny County Health Department, please fill out this ACHD Vaccine Event Form and someone from the Health Department will contact you.
